Print checklist

  • Use US Letter paper (8.5 x 11 inches) in portrait orientation
  • Print at 100% or Actual Size
  • Disable Fit to Page, Shrink, and similar scaling options
  • Verify the built-in rulers before anyone uses the chart
  • Reprint if the rulers are off

Use checklist

  • Mount the chart flat at eye level with bright, even lighting
  • Measure exactly 10 feet (3.05 m) from viewer to chart
  • Cover one eye without pressing on it
  • Read the smallest line that can be seen reliably
  • Treat the result as screening information, not a diagnosis

Why 10 feet

Because a true one-page 20-foot chart does not fit on home paper.

The PDF preserves the visual angle that matters for screening while staying practical to print. That is why the page is intentionally calibrated for 10 feet while keeping familiar 20/xx equivalent line labels.

Verify print scale

A printable chart is only useful if the physical geometry is correct. The rulers are not decoration.
